Retatrutide (LY3437943) is an investigational peptide that activates three receptors with a single molecule: GIP, GLP-1 and glucagon. It is not an authorised medicine and remains in clinical development.
The three pathways
- GLP-1 receptor — studied for glucose-dependent insulin secretion, glucagon suppression and central satiety signalling.
- GIP receptor — studied for insulinotropic effects and adipose-tissue lipid handling in preclinical models.

What the published evidence shows
Phase 2 trials (NEJM 2023; Lancet 2023) reported substantial effects on glycaemic and weight endpoints in the studied clinical populations, with a safety profile characterised mainly by gastrointestinal events. These are established, peer-reviewed findings.
What remains investigational
Phase 3 outcomes, long-term safety, and precisely how much the glucagon-receptor arm contributes are open questions under active study. Mechanistic claims from animal or in-vitro work should be read as hypotheses, not established effects.
